Instructions to the Head Master/Head Mistress

Before going fill the form read instructions carefully

Your 5 digit school SSC Code is your Username and Password... Last date of enroll teachers' data is 29/02/2020.

Your School 5 digit SSC Code is Username and Password. You can change the password once you enter in login to the website

1. Head Mater/Head Mistress only responsible person to fill this form.
2. First select mandal name from the dropdown menu.
3. Enter the Teacher/Head Master 7 digit Treasury id number.
4. Name of the Teacher should be in capital letter.
5. Designation is mandatory field and must select from the dropdown list.
6. If a teacher is directly School Assistant, then no need to fill the ‘Date of join as SGT’.
7. If a teacher is joined as school Assistant must enter joined date.
8. If your present cadre is Head Master/Head Mistress then enter ‘Date of join as Head Master’.
9. If a teacher teaching telugu medium only then select ‘ Telugu’. If a teacher teaching both mediums then opt ‘English’ medium .
10. Select number of years subject handling as on March 2021.
11. Select date of retirement column from the dropdown.
12. Take attention while typing your Bank A/c No.
13. Name of the Bank should be like SBI, Chittoor i.e enroll bank name with location.( Salary Account Number)
